Year-Round Adult Day Services Registration
We sincerely thank you for your interest in Adult Day Services!The weekday program is offered throughout the year at three Greater Cincinnati locations:
- Allyn Campus (Batavia)
- Drex Campus (Norwood)
- Parkcrest Campus (Western Hills)
Three-Step Enrollment Process
To register for Adult Day Services, please follow our three-step enrollment process.Step 1: Schedule a Tour
The first step to enrolling in Adult Day Services is to schedule a tour at your campus of choice.On your tour, you will have the opportunity to meet program staff and learn more about our daily schedule, program activities and services.
Step 2: Complete Enrollment Forms
The second step to enrolling in Adult Day Services is to complete our enrollment forms. During your tour, you will be provided with all necessary forms as well as other important information. If you misplace your enrollment forms or need an extra copy, you may print and complete the following:- Adult Day Services Application
- Master Medical Form (To be signed and completed by physician.)
Step 3: Attend an Intake Meeting
Before enrollment in Adult Day Services is complete, the participant and their support team (i.e. parent/guardian, home caregiver, SSA, etc.) must attend a required intake meeting with our program director.Completed enrollment forms can be submitted at the intake meeting or sent directly to Client Services:
