If you prefer not to register online, you will need to print out and complete the forms below. Upon completion, mail registration forms to:
Stepping Stones Client Services, 5650 Given Road, Cincinnati, OH 45243
1. Once a year, complete these forms to participate in any Stepping Stones activities:
- ANNUAL PARTICIPANT APPLICATION
- ANNUAL MASTER MEDICAL FORM (to be completed and signed by your doctor)
If you have submitted these forms within the last year, please check that the activities you are registering for will be covered within one year of the dates.
